File A Complaint

To file a complaint regarding program discrimination, you are required to complete Form AD-3027, known as the USDA Program Discrimination Complaint Form. This essential document is designed to gather the necessary information regarding your complaint and can be easily accessed through several channels. You can obtain the form online from the USDA's official website, visit any USDA office in person, call their dedicated helpline at (866) 632-9992, or by writing a letter to the USDA to request the form. If you decide to file your complaint via a letter, it is imperative to include specific information to facilitate the processing of your complaint. Your letter must contain your full name, current address, and telephone number. Additionally, you must provide a detailed account of the alleged discriminatory action. This account should include a clear description of the events you experienced, the individuals or entities involved, the date(s) on which the discrimination occurred, and any other relevant details that can help the Assistant Secretary for Civil Rights (ASCR) understand the context and seriousness of your claim.
